Syncope due to prolonged asystole: an indication for pacing?

Abstract

We present the case of a patient who developed prolonged asystole during an ophthalmologic operation leading to loss of consciousness. The patient reported two syncopal episodes in the past. Reproduction of asystole and syncope by head-up tilt testing confirmed the diagnosis of neurocardiogenic syncope. Whether the implantation of a pacemaker is beneficial in such cases remains questionable

Angiographic segmental narrowing of a saphenous vein bypass graft during diastole.

Abstract

Dynamic narrowing of native coronary vessels is widely known to occur during the systolic phase of the cardiac cycle. This phenomenon has also been reported to occur in coronary artery bypass grafts. However, to the best of our knowledge, only one case report exists of this event taking place in bypass grafts during diastole. We discuss the angiographic findings of a 73-year-old male who had undergone coronary artery bypass grafting surgery twice and experienced diastolic compression of his saphenous vein graft to the right posterior descending artery.

Acute myocardial infarction during a dobutamine stress echocardiography, in a patient with angiographically normal coronaries.

Abstract

We report a case of acute myocardial infarction during a dobutamine stress echo (DSE) study in a middle age man. Our patient experienced a severe crushing retrosternal pain at peak DSE study, at the time the ECG recorded an episode of non-sustained ventricular tachycardia followed by a persistent significant ST segment elevation in V2-V6 leads. Symptoms and ECG findings remained despite the use of intravenous (IV) nitrates and therefore thrombolysis was given. A subsequent coronary angiogram revealed only mild atheromatous changes but no stenosis.

Anomalous origin of right coronary artery from the mid-left anterior descending artery.

Abstract

We report a case of a right coronary artery (RCA) originating from the mid-left anterior descending artery (LAD) coursing around and in front of the pulmonary artery. The anomaly was discovered incidentally during cardiac catheterization. It is only the second case of such an anomaly ever described.
